A. REQUIREMENTS FOR SUBMITTING THIS PERMIT APPLICATION NOTE : THIS APPLICATION SHALL BE SUBMITTED TO THE ENGINEERING DEPARTMENT WITH THE FOLLOWING ATTACHMENTS. THE APPLICATION WILL BE DEEMED INCOMPLETE IF ALL OF THESE ITEMS ARE NOT PROVIDED. 1. A Certificate of Liability Insurance. Please note that Montgomery Township must be listed as an additional insured. 2. Scaled drawings of proposed work with pertinent details, usually on property survey or plot plan. 3. All parts of this permit shall be filled in. If there are items left blank, the permit shall not be processed. 4. If work will impact traffic, applicant shall provide a temporary traffic control in accordance with the Manual on Uniform Traffic Control Devices (MUTCD), Part VI, Latest Revision. 5. Applicant is to contact the Montgomery Township Engineering Department at (908) 359-8211, a minimum of 48 hours prior to commencing work, to schedule an inspection. 6. New Jersey Law requires anyone digging to schedule a valid utility mark out call at least three full business days (excluding weekends and holidays) prior to beginning work. This includes excavators as well as property and homeowners’ contractors. Contact New Jersey One Call at 811 or (800) 272-1000 any time, day or night. If directed by the Township Engineer, or his authorized representative, a site meeting will be scheduled ten (10) working days prior to commencing work. 8. Inspection fees and bond fees apply and are determined during application review. Bond fees are eligible for release with applicant’s written request (email preferred) 6 months after final inspection by Engineering Department; inspection fees are non-refundable. 9. Sump pumps, interceptor drains, or other front yard drainage discharges shall be tied into the storm sewer system where possible. 10.Restriction on permits: No permit will be issued within 3 years of road resurfacing or within 5 years of new construction/reconstruction. B. BACKFILL REQUIREMENTS Unless otherwise directed by the Township Engineer or his authorized representative, the following specifications shall govern the backfill in excavated trenches: a) All roadway openings shall be backfilled with Type 5, Class A stone and shall be tamped in six (6) inch layers with a mechanical tamper. A six (6) inch sand cushion or ¾” clean stone may be applied around pipes, conduits, cables, wires, etc., for protection of such facilities. b) All other openings shall be backfilled with suitable excavated material and shall be tamped in six (6) inch layers with a mechanical tamper. C. TRENCH AND PAVEMENT REPAIR REQUIREMENTS Repairs to trenches and other excavations shall be in accordance with the following conditions: 1.All trenches and/or openings in the roadway shall be permanently repaired promptly after the completion of work or as directed by the Township Engineer. Backfill and restoration requirements subject to review by the Township Engineer during the application process. 2.The applicant shall maintain temporary patches in acceptable condition. Deficient patches shall be repaired within twenty-four (24) hours of notification by the Township Engineering Department. 3.Once the permanent base course paving is installed, the top course paving shall be installed within seventy-two (72) hours. 4.All openings in grass areas must be restored with topsoil, fertilizer, seed, and mulch. D. SIDEWALK, APRON AND CURB REPAIR 1. Any sidewalk damaged or disturbed during the excavation shall be replaced, according to the Land Development Ordinance Section 16-5.14c.3. 2. No repairing of damaged or reuse of disturbed sidewalk will be permitted. 3. Concrete shall not be mixed by hand on the site; a concrete truck from a concrete plant shall deliver it. The mix shall meet the specifications of the ordinance referred to above. 4. Curb repairs shall be in accordance with Section 16-5.14b, of the Land Development Ordinance. If curb is removed additional Escrow Fee shall be required by Ordinance. 5. All work shall conform to the current New Jersey Department of Transportation Specifications for Road and Bridge Construction, as amended. E. DISCHARGE HEADER SYSTEM 1. All discharge header systems shall be constructed according to the Land Development Ordinance, (Section 12-11.2, Program Established) and (Section 16-5.2a, Drainage). 2. Pipe with cleanouts shall be installed according to detail sheet # D1.13.
